To schedule an appointment at The Chiari Institute, please follow these steps:

1. Print the following forms:
    
   • Questionnaire
   • Tethered Cord Syndrome Screening Tool
      
2. Fill them out
    
3. Mail them to TCI at:
   
         865 Northern Boulevard
         Suite 302
         Great Neck, NY 11021
         USA

* Please write "New Patient Info enclosed" on the envelope.

* Be sure to include a copy of your insurance card(s) with the questionnaire.
